Trust Me I’M A Doctor…. Working towards a Healthy Prescriptive Consent According to Decision Theory | The consent process is a complex relationship between rational cognitive thinking and emotional processes [1] (page 62) that can alter the choices that are being made and can shape probabilities of outcomes [2]. The patient or “lay person” is presented to the cardiac surgeon “expert” by means of a referral letter from a cardiologist who in turn saw the patient following referral from a General Practitioner. Both the patient and the surgeon have a general understanding of the need to act [3]. There is an expectation of surgery but the objectives and goals of surgery have not been defined. The consent process involves clarification of those objectives, exploration of alternatives and the addition of some structure to the decision process. This, however, is subject to natural biases or “heuristics” and especially subject to framing. I hope to explore these biases from the perspective of the patient ‘lay’ customer and the consultant surgeon or ‘expert’ and examine the consultation process using prospect theories. I hope to look beyond these heuristics and offer prescriptive alternatives that may go some way towards an ideal consent. Trust Me I’M A Doctor…. Results of erosion processes of brown mountainous-steppe soils of the southeastern part of the Small – Caucasus |Brown mountainous-steppe soils arranged in the form of spots in separate forest edges and meadows within the zone of brown mountainous- forest soils. The lower boundary of these soils coincides with the upper boundary of gray-brown mountainous soils. The problem of steppification of forest soils was widely covered in the writings of the V.V. Akimsev [1], S.A. Zakharov 1937, S.V.Zonn [2] and other researchers. Information on Azerbaijan, on the processes of steppification of mountain-forest soils is available in the works of G.A Aliyev [3], ME. Salayev 1966, K.A. Alekberov [4], Sh.H Hasanov [5], and others. Man in Outer Space without Space Suite | The author proposes and investigates his old idea – a living human in space without the encumbrance of a complex space suit. Only in this condition can biological humanity seriously attempt to colonize space because all planets of Solar system (except the Earth) do not have suitable atmospheres. Aside from the issue of temperature, a suitable partial pressure of oxygen is lacking. In this case the main problem is how to satiate human blood with oxygen and delete carbonic acid gas (carbon dioxide). The proposed system would enable a person to function in outer space without a space suit and, for a long time, without food. That is useful also in the Earth for sustaining working men in an otherwise deadly atmosphere laden with lethal particulates (in case of nuclear, chemical or biological war), in underground confined spaces without fresh air, under water or a top high mountains above a height that can sustain respiration. Oncolytic Virotherapy and Multiple Myeloma | OV immunotherapy is an important emerging cancer treatment modality which uses native or genetically modified viruses to destroy cancers. MM is perhaps the ideal malignancy to target using OVs because of the virus permissivity of neoplastic plasma cells and the diseaseassociated immune paresis. OVs have the potential to mediate both tumor debulking and residual disease elimination avoiding the complex and prolonged current therapy for multiple myeloma. Oncolytic Virotherapy and Multiple Myeloma. OV immunotherapy is a new form of cancer therapy that uses native or genetically modified viruses to selectively enter, replicate and lyse tumor cells without causing harm to normal surrounding tissues [1]. OVs can infect both normal cells and cancer cells, but they demonstrate a preference for cancer cells due to aberrant cell processes and a cancer-induced downregulation of the host antiviral response [2]. Normal cells respond to virus infection by downmodulating their metabolism and/or by undergoing apoptosis, thereby inhibiting virus propagation [3]. OVs also release cancer antigens causing an immune attack against the infected malignant cells [1]. Thus, OVs offer the attractive therapeutic combination of tumor-specific cell lysis together with immune stimulation, therefore acting as potential in situ tumor vaccines. Hypertrophic Cardiomyopathy….in Search of its Own Identity | Hypertrophic cardiomyopathy is a disease of muscle of heart. The incidence is 0.2% in general population. Only 15% of total patients are being diagnosed and large no of patients are yet to be screen-positive ,about 85%.The motive of this article is to activate medical practioner to diagnose the disease early and prevent the complication ,one of them is sudden death. Incidence of sudden death is about 2% to 4%. Hypertrophic Cardiomyopathy….in Search of its Own Identity. In 55% of patients it occurs in autosomal dominant conditions. It occurs due to mutation in one of the nine gene coding protein of cardiac sarcomere. B-MHC, Cardiac Troponin T, Myosin binding protein C, Alpha-Tropomyosin. This will result in cardiac Myocytes Hypertrophy, disturbances in alignment of muscles of heart and disruption of electrical functioning of heart. Its incidence is 33% in close relative of patient. Hexadactyly: A Rare Case | A 2-year-old Caucasian female patient, referred to our Department, presented on physical examination a symmetrical duplication of the 5th finger in both hands and feet. (Figure 1) Bone structures could be also palpated and observed at X-ray exam (Figures 1 & 2). Clinical family history was negative for malformation disease or teratogenic drugs taken during pregnancy. Furthermore the child was not suffering from other associated malformations or organ dysfunction, the genetic examinations showed a normal karyo type and the absence of known mutations. She had no other anomalies and growth and development were normal. Radiographic examination showed six sets of phalanges with five metacarpals Figure 1; the last finger did not have an associated metacarpal in both hands but on the left the proximal phalanx of the last finger appears fused with the fifth metacarpal instead on the right hand the sixth finger is attached by a skin bridge only. The long bones of the arms were normal. According to Stelling and Turek classification, the right hand can be classificated as Type I, the left one as Type II Hexadactyly: A Rare Case. Total Phenolic Content of Three Samples from the Powdered Stem Bark of Detarium Microcarpum Guill. and Perr. (Fabaceae) | Preliminary processing of the plant material was done. The stem cuttings were debarked and the barks dried in an open air under shade. The macroscopical examinations were done. The dried plant materials were powdered. Three different samples of the plant extracts/fraction were used in the determination and the UV-Visible Spectrophotometric Analysis method was adopted. Tannic acid was used as the standard. The absorbencies were measured using the Jenway 630 UV-visible spectrophotometer at 570nm against water: methanol blank. The various absorbance obtained were plotted against the corresponding concentrations to obtain a standard calibration curve. The Caspian Littoral Strip in Azerbaijan is concerned with the State of Ecology of Heavily Exposed Metals | Pollution of the environment has become one of the most important tasks of our time; a special role belongs to heavy metals, which have the ability to accumulate in soils and through them to enter food products, while contributing to soil degradation. Absheron Peninsula is an anthropogenic load; it is the most tense in Azerbaijan. The peninsula is located on the western shore of the Caspian Sea and is the southeastern tip of the Greater Caucasus Range, occupying a total area of 200 thousand hectares. In the middle part its width is 28 km, and the length from east to west is 62 km. Absolute marks of hypsometric level of the surface from 25 to 300-350 m above sea level. The relief is characterized by soft, smoothed and weakly dissected, representing a slightly wavy plain, with a progress toward the west, where it gradually elevates, which is explained, first of all, by the young age in geological terms of the relief and the insignificant height of the peninsula above the basis of erosion.
