Primary Intimal Pulmonary Vein Sarcoma with Expansion to the Left Atrium | We have recently treated a patient with pulmonary vein sarcoma. It is a 65 year old women who was admitted to the hospital because of the chest pain with propagations down her right arm, followed by suffocation and coughing out content with traces of blood and febrility >38°C. The primarily suspected pulmonary embolism was ruled out after the diagnostic heart CT scan and transthoracal and transesophageal echocardiogram which verified the existing of a tumor mass in the left atrium. The patient underwent an urgent surgery and the tumor mass was removed surgically from the upper right pulmonary vein. The path histological diagnoses revealed pulmonary vein intimal sarcoma. Pulmonary vein intimal sarcoma is one of the rarest sarcoma subtypes. To our knowledge, there is only one published case of such pathology in the literature. Laparoscopic Adrenalectomy Formetastatic Adrenal Mass as Diagnosis of Suspected Lung Tumor: A Case Report | Adrenal metastases at time of initial diagnosis occur in less than 10% patients with non small cell lung cáncer. To an adrenal mass and a pulmonary nodule, adrenal ectomy should be performed in suspected adrenal metástasis from a primary lung tumor. A 60-years old male, ex-smoker since seven month ago, COPD, and colonic polyps, presents fever of unknown origin. A computed tomography scan of the thorax and abdomen demonstrated a pulmonary node of 15 mm in the upper right lobe (Figure 1) and a left adrenal mass of 8x9x8 cm with out defined fat planes of the left kidney upper pole and the left renal vein pedicle (Figure 2). Bronchial biopsy was no posible with Flexible bronchoscopy. The core needle biopsy showed the tumoral in filtration. Were sected the left adrenal mass after ligation of the left adrenal vein by laparoscopic approach with four trocars and with out complications. The tumor was totally resected with the spleen and the left kidney because they seemed in filtrates; we left a drain at the upper left cuadrant. The operating time was 200 minutes. The resected specimen include a metastatic mass of 12x10x8 cm, the spleen of 10×6 cm and the left kidney of 9x6x4 cm (Figure 3 & 4). Microscopic examination confirmed metastatic adenocarcinoma originating from the lung. The spleen and the left kidney had not tumor in filtration. Reconstruction of a Worn or Mutilated Dentition with the Utilization of an over Denture Appliance for a Patient with Enamel Deficiency – A Case Report | Evidence suggests that the retention of mandibular roots for the preservation of bone in patients by utilizing an over denture is a conservative approach for a fixed and removable prosthetic reconstruction. A young adult with worn and fractured dentition exhibited severe loss of function and esthetics, exhibiting both occlusal discrepancies as well as esthetic problems. The treatment was preventive as well as conservative in nature. It was recommended to preserve endodontically treated teeth in order to preserve the bone in young patients with a severely worn, non-restorable dentition. By reviewing the current literature, several aspects of patient selection and treatment planning are discussed. This case report suggests that patients can benefit from a fixed and removable maxillary and mandibular prosthetics by using over denture stud attachments, preserving the endodontically treated roots in order to maintain bone. Does MIDH Inhibitors is a Promising Therapy? | IDH are key metabolic enzymes that play a role in mitochondrial oxidative phosphorylation, glutamine metabolism, lipogenesis, glucose sensing, and regulation of cellular redox status. Mutations in IDH1 and IDH2 proteins catalyze the reduction of α-KG to 2-HG. The abnormal production of the oncometabolite 2-HG promote tumorigenesis. Depending on the associated genomic aberrations and the cellular context, the oncogenic potential of IDH1/2 mutations ranges from an initiating event – promoting transformation – to a secondary oncogenic event conferring selective advantage to cancer cells. Morphological and Technical Factors Associated with Complications and Recanalization of Pericallosal Artery Aneurysms after Endovascular Treatment | We present a series of 31 patients who underwent endovascular treatment due to a pericallosal artery aneurysm. To our knowledge, this is the largest series of patients with pericallosal aneurysms treated by endovascular approach reported so far. The distal location and generally small size of these aneurysms both make them difficult for surgical and endovascular treatment. Our aim was to try to define the risk factors for recanalization and complications of treatment. Various factors were analyzed, such as the patient’s age, occurrence of subarachnoid hemorrhage, aneurysm size, morphology/neck width and side of the aneurysm, incorporation of arterial branches, dominant ipsilateral A1 segment and other anatomic variations, presence of other aneurysms, type of coils used for treatment, the need for stent implantation and the initial degree of occlusion. Our rate of complications of 9.7% and recanalization of 16% were relatively high compared to other aneurysm sites. Relatively large aneurysms and those which incorporated branches were more prone to recanalization. Pericallosal aneurysms in our study were however very morphologically and anatomically heterogeneous, making our sample size insufficient to show the impact of these factors on complications. Is Diverticulitis Becoming a Younger Population Disease: A Case report on Large Pelvic Abscess as a Result of Diverticulitis in a Patient with a High BMI | Diverticular disease is still a very significant disease in the modern world, affecting 20 to 60% of the population. Acute diverticulitis will develop in 25% patients with diverticular disease. The prevalence in the western world is on a constant rise, with a significant increase in the incidence of diverticulitis in the younger population. Approximately around 20% of patients in western population with acute diverticulitis are under the age of 50. The patient was admitted to our emergency department, and after preoperative management, she was surgically treated for a massive pelvic abscess, and intestinal obstruction, caused by a complication of acute diverticulitis. Initially, drainage of pus and a colostomy was performed. After 12 weeks of follow-up, that included a colonoscopy, and a contrast CT, an elective left hemicolectomy was made, with a colo-rectal end-to-end anastomosis. Esophageal Choke and its Management in a Thorough Bred Horse | We describe the clinic case of a thoroughbred horse with esophageal obstruction that presented a fatal outcome, possibly attributed to inadequate management in which excessive use of the nasogastric tube to push the contents was responsible of ulceration and inflammation of the esophageal mucosa with posterior cicatricial reaction and irreversible stenosis of the organ. The importance of medical therapy, based on analgesics, anti-inflammatory, sedatives (alpha 2 agonists), oxytocin, is highlighted. In addition, the use of nasogastric tube only to dissolves the obstructive content using soft pressure water and not to push content. A Conceptual Review on Micro Bubbles | Microbubbles system is a newly invented non-invasive technique in which, in the presence of ultrasound waves or harmonic sounds micropores shrinks and agitated to produce micro bubbles within the applied area. These bubbles have the capability to target any perfused tissues within our body. Recently it was postulated that due to its narrow size range (1 to 10μm vesicular diameter) it can get easily engulf by phagocytic blood cells which can be very effective on lymphocytic cancer. Micro bubbles are prepared by using mechanical agitation, ultrasonication, pressurized gas-liquid mixing system etc. Micro bubbles are made up of a monolayer of protein shells, lipid shells, surfactant shells, polymer shells, Polyelectrolyte multilayer shells. Initially, microbubbles are used as diagnostic tools but due to its versatility of uses and properties of surface conjugation of proteins, genes, micro molecular nutrients, drugs slowly it’s becoming a therapeutic delivery tool. ALBUNEX® (GE Healthcare) was the first albumin conjugated micro bubble which was been approved by US-FDA. Recently importance is been given for brain targeting using Micro bubble carrier system. It was observed that using passive and active transport micro streamed bubbles can able to transport through Blood Brain Barrier (BBB). Using Simulation Technology and The Root Cause Analysis Process to Assess Nurse’s Attitude toward Patient Safety | Root cause analysis (RCA) provides an important opportunity for healthcare workers to identify the underlying factors that may contribute to medical errors or sentinel events and subsequently prevent their recurrence. Because of its efficacy, The Joint Commission’s (TJC) recommended RCA framework together with In-situ simulation were used to assess nurses’ attitudes toward patient safety immediately after simulation training and six months later. Thirty-three nurses from a hospital setting participated in RCA sessions which included: a lecture, a simulated patient scenario and debriefing. A 36-item “Safety Attitudes: Frontline Perspectives” survey was given before and after each session and six months’ later. Twelve (39%) nurses responded favorably to patient safety initiatives on the first survey, and twenty-nine (89%) responded favorably after six months (p=0.001). A significant number (p=0.003) of nurses perceived that the institution still needs to improve some patient safety measures.
