Does Single Locked Plating Provide A standard treatment Modality for Bicondylar tibial Plateau Fractures? A prospective study | Bicondylar tibial plateau fractures present a therapeutic challenge to the orthopaedic trauma surgeon, both in terms of the osseous injury as well as the concomitant soft-tissue insult. Double Plating with single incision or dual incisions provides more insult to the compromised soft tissue. However, single locking plate combines the technical advantages of an angular stable plate with those of the modern biological plating technique. The Study was held at Razi Orthopedic hospital in Kuwait. Between May 2012 and November 2013, 20 patients with a mean age of 37.65 years (Range from 24-57 years) with Bicondylar tibial plateau fractures with or without metaphyseal extension. Patients were diagnosed clinically, checked with standard X-rays; CT was done for all cases. Patients were treated by single lateral anatomically contoured locked plate through LISS or Polyaxial locking plate systems with or without additional screws from medial side. Radiological evaluation and functional assessment was done according to the Rasmussen Knee score. Patients were followed-up for an average of 12 months. Assessing the Quality of Perineal Auscultation for A Noninvasive Diagnosis of Urinary Bladder Outlet Obstruction | The current standard method for diagnosing bladder outlet obstruction in patients with Lower Urinary Tract Symptoms (LUTS) is a pressure-flow study. Electronic perineal auscultation, i.e. recording sound with a contact microphone behind the scrotum during voiding, might be an easy-to-use non-invasive alternative. Of major importance for the clinical applicability of perineal auscultation is the quality of the sound recording. In a clinical population of 74 male LUTS patients we studied two measures for assessing the quality of electronic perineal auscultation: a Normalized Correlation Coefficient (NCC) and Signal-To-Noise Ratio (SNR). We compared these measures with the visual assessment of the electronic auscultatory signal by three experienced observers. The agreement between the experienced observers was expressed using the Weighted Kappa statistic. Both quality measures were compared to the combined visual assessments using the Kruskal Wallis test and the usefulness was evaluated using ROC-analysis. Plasma Lipid Profile and Uric Acid in High Fat Fed Female Rats Treated with Oral Contraceptive | A high fat diet is widely known in scientific consensus to be associated with abnormal lipid and uric acid metabolisms. The dose-related influence of Combined Oral Contraceptive (COC) in both human and animal models on blood lipid parameters is still controversial. Therefore the aim of the study is to investigate the effect of high fat diet (HFD) and two different doses of a COC on plasma lipid profile and uric acid in female wistar rats. Rats were divided into vehicle treated, high dose COC (HCOC, receiving 7.5μg Levonegestrel/1.5μg of estradiol), low dose COC, (LCOC receiving 1.5μg Levonegestrel/0.3μg of estradiol), High Fat (HF receiving 20%w/w of vegetable oil (turkey brand) supplemented feed), HF+HCOC (receiving both 7.5μg Levonegestrel/1.5μg estradiol and HFD) and HF+LCOC (receiving 1.5μg Levonegestrel/0.3μg of estradiol and HFD). Rats were given distilled water, HCOC, LCOC, HFD, HCOC+HF and LCOC+HF for 8 weeks. When compared with vehicle group, HCOC treatment led to a significant increase (P<0.05) in total cholesterol (TC). LCOC treatment also caused a significant rise (P<0.05) in total cholesterol, triglyceride and uric acid. Rats fed HFD showed a significantly higher (P<0.05) total cholesterol and Total/HDL-C ratio. HCOC+HF administration led to a significant elevation (P<0.05) of total cholesterol and Low density lipoprotein (LDL-C). LCOC+HF administration produced a significant increase in total cholesterol, LDL-C, Triglyceride/HDL-C ratio, Total/HDL-C ratio, LDL-C/HDL-C ratio and uric acid. In conclusion, combined oral contraceptive and high fat diet administrations resulted in an elevated total cholesterol and uric acid. The effect on Triglyceride/HDL-C, Total/HDL-C, LDL-C/HDL-C and uric acid was dependent on dose of the contraceptive, the effect on TC and LDL was not dose- dependent. Administrations of low dose of combined oral contraceptive and high fat diet may produce a greater cardiovascular risk than administration of high dose of combined oral contraceptive and high fat diet. Ghost Cell Odontogenic Carcinoma-a review of the literature | Ghost Cell Odontogenic Carcinoma is an extremely rare odontogenic tumor with unclear etiology. Although known by many names, the basic histologic identifying feature includes the identification of the ghost cells and the malignant changes associated with the cells. These tumors show a spectrum of growth from a slow-growing locally invasive tumor to a highly aggressive, rapidly growing, infiltrative tumor. Therefore, it is prudent to perform a wide local excision with histologically clear margins. Is seen associated with squamous cell carcinoma, radiotherapy also should be included in the treatment plan. Long term follow-up is highly recommended. β-HCG as a Tumor Biomarker with Special Reference to Breast Cancer | Human chorionic gonadotropin (HCG) is a placental protein hormone initially secreted by cells (syncitiotrophoblasts) from the implanting conceptus. It supports the ovarian corpus luteum, endometrial lining and consequently maintains pregnancy [1]. The hormone can be detected in blood and urine, as it is the basis of many pregnancy tests. HCG plays many other roles including stimulating the onset of fetal gonadal steroidogenesis, promotion production of corpus luteal progesterone, immuno-suppression and blockage of phagocytosis of invading trophoblast cells, cytotrophoblast differentiation, growth and differentiation of fetal organs [2]. It is well known that a biomarker is any molecular and chemical changes that can be measured and used to study normal or abnormal process in the body. A tumor marker, like a biomarker, is a naturally occurring substance in the body. A tumor marker can be made by cancer cells or by the body in response to cancer. Ascitic Type of Abdominal Tuberculosis in a 40 Year Old Female | Disseminated tuberculosis can cause a lot of damage to an individual if there is no early diagnosis. In Africa, about 70% of patients with abdominal tuberculosis present with ascites and most of these cases are missed diagnostic cases. The case in question is of a 40 year old women from one of the rural areas of Zimbabwe who had previously presented with severe coughing but had denied sweating at night and the sputum results were negative on screening. After 2 weeks she presented again at the clinic with a swollen abdomen and the X-ray showed flooding of the lungs and difficulty in breathing. Disseminated tuberculosis can cause a lot of damage to an individual if there is no early diagnosis. In Africa, about 70% of patients with abdominal tuberculosis present with ascites and most of these cases are missed diagnostic cases [1,2]. The patient presents with a swollen abdomen containing many litres of strawcoloured fluid. The fluid accumulates as the result of large numbers of miliary tubercles on the peritoneum. The only certain way to make the diagnosis is to do a minilaparotomy (‘’minilap’), which will also enable you to diagnose cirrhosis, peri portal fibrosis (due to Schistosomiasis mansoni), carcinomatosis of the peritoneum and hepatoma (usually with cirrhosis). Experts can usually diagnose miliary tuberculosis with their naked eyes; but even they can be wrong, so take a biopsy of his parietal peritoneum and/or his liver. Hemophagocytic lymphohstiocytosis: A Case Report | Hemophagocytic lymphohistiocytosis (HLH) is a life threatening hyper inflammatory syndrome occurring due to ineffective immune process. It can be genetic (familial form) or secondary (acquired form). We present a case of HLH in a 50 year old male who presented with pancytopenia and huge platelet clumps on peripheral smear which subsequently leads to diagnosis after correlation with clinical, hematological and biochemical findings. Hemophagocytic syndrome with special mention to its secondary form is a rare syndrome which occurs after strong immunologic activation and release of cytokines characterized by unrestricted activation of T lymphocytes and macrophages invariably leading to death in absence of treatment [1,2]. It is associated with certain clinical and hematological abnormalities of which pancytopenia, hyper ferritinaemia, hepatosplenomegaly, fever and hemophagocytosis in bone marrow were observed in our case. Follicular Dendritic Cells Sarcoma in Accessory Salivary Glands | Follicular Dendritic cells sarcomais a relatively rare entity with intermediate malignancy. Their evolution is characterized by the frequency of local and/or remote relapses. Due to their rarity, the treatment is not consensual in the literature. We report a rare case of follicular dendritic cells sarcoma of accessory salivary glands in a 45-year-old patient whose progression is favorable after surgery followed by chemo-radiotherapy and review of the literature. Follicular Dendritic cellstumors are rare entities (less than 1% of malignant salivary gland tumors). Only a few cases have been reported in the literature [1-2]. In addition to their rarity, these tumors shows difficulties in the diagnosis of certainty and in the optimal the rape utic strategy. We report an exceptional case of follicular dendritic cell sarcoma of submaxillary accessory salivary glands in a 45-year-old man and discuss the therapeutic strategy based on data from the literature.
